Today, dental implants are used to support all different kinds of restorations (fixed vs removable, single tooth replacement vs partial or full arch restorations). As a consequence, pre operative diagnostics are considered to be the foundation of a precise as well as a restoration driven implant placement. Intra operatively, surgical stents and other visual aids are routinely used to transfer the planned 3 dimensional implant position into the surgical field. However, traditional surgical templates are mostly used to re check drilling steps and the finally established implant position only on a visual basis. Therefore, local anatomic conditions (bone defects, etc.), limited surgical skills, or simply a poor visual judgement may still lead to a compromised implant positioning. In contrast, a real guided implant surgery relates to a new generation of surgical template and offers a precise transfer of the planned implant position into the surgical field. Thereby, pre operative planning and production of surgical templates are either model based (for single tooth and small partial cases) or computer generated (long span partial and edentulous cases). Overall, guided implant surgery reduces chairside time, morbidity, post operative pain and swelling, and may also offer a flap less surgical approach or an immediate restoration protocol. Hence, guided implant surgery may also be considered as a next level in quality assurance. This lecture discusses todays different options using either model based or CT derived templates for guided implant surgery.

    The goals of contemporary restorative dentistry and prosthodontics should be (1) the maintenance of the vitality of the teeth to be restored and (2) the maximum preservation of sound tooth structure. The core idea of the biomimetic principle in restorative dentistry and prosthodontics is that the intact tooth in its ideal hues and shades, and perhaps more importantly in its intracoronal anatomy, mechanics and location in the arch, is the guide to reconstruction and determinant of success. The approach is basically conservative and biologically sound. The application of the biomimetic principle involves returning all of the prepared dental tissues to full functional by the creation of a hard tissue bond that allows functional stresses to pass through the tooth, drawing the entire crown into the final functional biologic and esthetic result. Unlike traditional alloy restorations and cemented crowns, novel design indirect composite resin/porcelain adhesive restorations are stress distributors and involve the crown of the tooth as a whole in supporting occlusal force and masticatory function. Indirect bonded restorations, also provide the practitioner with a noninvasive and social restorative tool owing to the minimum maintenance costs. One can anticipate considerable improvements such as in the treatment of crown fractured and worn down teeth, which should reduce the need for preprosthetic interventions (e.g. root canal therapy and crown lengthening) and the use of intraradicular posts.

    Surgical placement of oral implants is governed by the prosthetic design and by the morphology and quality of the alveolar bone. Often, implant placement may be difficult, if at all possible, due to alveolar ridge aberrations. In consequence, prostheticly dictated implant positioning commonly entails bone augmentation procedures. One objective of our laboratory is to evaluate the biologic and clinical potential of bone morphogenetic proteins (BMPs) including rhBMP 2, rhOP 1/rhBMP 7, rhGDF 5, other candidate biologics, bone biomaterials, and devices for alveolar ridge augmentation and implant fixation. This presentation will discuss the unique biologic potential, the clinical relevance and perspectives of BMP technologies for alveolar bone augmentation and oral implant fixation, in particular the development of a unique bone inductive oral implant. This presentation will also address merits and explain short comings of current treatment protocol including bone biomaterials and guided bone regeneration (GBR). BMPs have an unparalleled potential to augment alveolar bone and support implant osseointegration and long term functional loading. Inclusion of BMPs for alveolar augmentation and osseointegration will not only enhance predictability of existing clinical protocol but radically change current treatment paradigms. Inclusion of the bone inductive oral implant in the treatment panorama may make 'grafting' and GBR procedures altogether obsolete.

However, replacing single, small sized teeth like upper lateral and lower incisors is often limited due to available mesio distal space. The diameter of conventional, small platform 2 piece implants is mostly around 3,3 to 3,5 mm. With regard to neighbouring teeth, a minimal distance between implant and root of around 1,2 1.5 mm is recommended in order to maintain interproximal attachment levels at natural abutments. Thus, an overall mesio distal gap size of at least 6 to 6,5 mm is needed when selecting conventional implants. With the introduction of 1 piece implants, now smaller diameters down to 3 mm are possible without jeopardizing the long term fracture resistance. In combination with an adequate surgical and prosthetic protocol, these 1 piece implants represent a new treatment option for restoring small sized gaps with a mesio distal width of down to 5 mm.

To reliably reflect the prognosis, studies must conform to design and methodology criteria consistent with an acceptable level of evidence. These criteria are met by only a few studies on retreatment and apical surgery.     LECTURE: THE USE OF MTA IN CLINICAL AND SURGICAL ENDODONTICS

    Recently, Dr. Mahmoud Torabinejad of Loma Linda University, California, has developed a new cement named Mineral Trioxide Aggregate which appears to have all of the characteristics requested of the ideal cement to seal pathways of communications between the pulp and the oral cavity (mechanical and carious pulp exposures), and between the root canal system and the periodontium (iatrogenic perforations, open apices, resorbed apices, root end preparations).

    MTA is an endodontic cement that is extremely biocompatible, capable of stimulating healing and osteogenesis, and is hydrophilic. MTA is a powder that consists of fine trioxides (Tricalcium oxide, Silicate oxide, Bismute oxide) and other hydrophilic particles (Tricalcium silicate, Tricalcium aluminate, responsible for the chemical and physical properties of this aggregate), which set in the presence of moisture. Hydration of the powder results in formation of a colloidal gel with a pH of 12.5, that solidifies to a hard solid structure in approximately three four hours. This cement is different from other materials currently in use because of its biocompatibility, antibacterial properties, marginal adaptation and sealing properties, and its hydrophilic nature.

    The characteristic that distinguishes MTA from other materials used to date in endodontics is its hydrophilic properties. Materials used to repair perforations, to seal the retro preparation in surgical endodontics, to close open apices, or to protect the pulp in direct pulp capping, are inevitably in contact with blood and other tissue fluids. Moisture may be an important factor due to its potential effects on the physical properties and sealing ability of the restorative materials. As shown by Torabinejad et al., MTA is the only material that is not affected by moisture or blood contamination: the presence or absence or blood seems not to affect the sealing ability of the mineral trioxide aggregate.

    During the presentation, several cases of treatment with MTA will be showed. Aim of the presentation is to show the indications and the technique for the correct use of this relatively new material: how to seal an open apex, how to repair a strip perforation under the microscope, how to protect a pulp exposure to perform a direct pulp capping, how to use MTA during surgery to seal the retroprep.

    In conclusion, the viewers should be able to appreciate the advantages of this material, which made our treatments more predictable and gave us the possibility to save teeth otherwise condemned to extraction.

    his presentation is the second of 4 modules related to treating the fully edentulous patient with removable overdentures. It will focus on the use of a jaw relation recording device (or tracing device) which will give the clinician new accuracy in procurring centric relation.

    The patient will generate a gothic arch tracing for us that will enhance our ability to be more accurate in our occlusal relationships. We will also illustrate the use of locator attachments: how they are chosen, and how their retentive capabilities are utilized.

    We will also show the use of strengthening metal frameworks for our denture bases, and how they add to the longevity of the denture base, and how it also allows us to produce a paletteless design in the maxillary overdenture. And, finally, we will illustrate the use of the pickup procedure intraorally, allowing us to pickup the locator attachment with a new material. Instead of using the older Methyl Methacrylates, it utilizes a lighter material that is cleaner, quicker, and more efficient.

    This module will dovetale in from the previous module of the series to demonstrate how we can successfully provide implant overdentures for our patient clientele.

    The treatment of the fully edentulous patient with computer assisted tomography, computer assisted surgical planning software, and a guided surgical technique using a stereo lithography surgical template for implant placement has five year clinical studies reported. Initial issues with esthetics and fit of the prosthesis are concerns. This presentation will discuss the treatment of the fully edentulous arch, treatment factors, and treatment options. At the conclusion of the presentation a demonstration will be performed treatment planning the edentulous patient using a surgical planning software program.
