CLINICAL CASE STUDIES VOLUME THREE: TRAGEDY IN PSYCHIATRY

Our clinical cases series have been mainly short cases with several on the disc. Clinical Cases Three is different in that there is only a single case concerning one patient and her family.

Working in Psychiatry presents unique challenges and this highlights some of them. There are lessons here for all mental health care workers, especially students.

The presentation lasts nearly one hour and includes interviews with the patient and her mother. The diagnostic problems are highlighted and discussed with senior staff. One of the commonest worries students have is knowing what to do when disaster strikes and this is fully explored. Sometimes there are no easy answers.

    PSYCHOLOGICAL DEFENCE MECHANISMS

    Beginning with denial, the first case depicts a normal reaction to loss; a wife cannot believe her husband is dead. Another example shows a smoker in a hospital bed. He has gangrene of his right foot; it is clear to the visiting doctor that it will require amputation below the knee the same as his left foot. He denies this and tries to convince us that he is getting better - yet amputation tomorrow will be the evitable outcome. A very thin, anorexic patient tells us that she needs to be six or seven pounds lighter is another illustration of denial.

    Projection, regression and repression are presented in a similar manner - progressing from the normal use as a coping mechanism to the more pathological defence mechanism.

    TEACHING ABOUT ALCOHOLISM
